Please follow the steps below.
​
1) Print off the registration for and complete it by filling out the document (neatly and legibly)
​
2) Attach a photocopy of your child's Roman Catholic Baptismal Certificate
​
3) Enclose a cheque of $60 made payable to "St. Alphonsus Church"
3) Mail the registration form to the parish at:

Should you wish to receive the sacrament of Confirmation at St. Alphonsus Church, one must be in or above the seventh grade. In order to receive this sacrament , the Confirmation Registration form must be completed and returned to the Parish Office. Additionally, please read the package entitled "Choosing a Confirmation Sponsor".
